Outstanding Community Service by an Individual - Award Nomination Form

Embracing both the University community as well as the local Denver/Aurora community through involvement and service is a priority to many students at the Anschutz Medical Campus. Through commitment to enhancing our community via acts of service, the individual student meets the needs of the community, engages in hands-on learning, and finds personal fulfillment.

Eligibility:
Any student enrolled as a degree seeking student in a degree program at the Anschutz Medical Campus who has provided outstanding community service through their own efforts as an individual, outside of any contributions they may have made as a member of a student organization.

Criteria:
The award recipient must have a continuing commitment to community service as reflected in one or more programs and initiatives, which are responsive to community needs, and exhibit a new or continuing partnership with the community. (Note: Students whose service is limited to, or was initiated by participation in a course for academic credit are not eligible for this award.)
